Square⇔hexagon Dissection
The dissection can be found on p.18 of the book Recreational problems in geometric dissections …,
and HAKMEM, Item 85 (see e.g. here)
seems to attribute its origin to Scientific American, Nov. 1961, p.164.
There are at least three other five-piece square⇔hexagon dissections known: those of Busschop and of Theobald, and the one found also on p.18 of the above book.
